Student Information
- Parent's name
- Student's full name

- Potential graduation date
School Information
- School name
- School address
- Phone number for school/parent
Course Information
For each course, provide:
- Course title
- Letter and number grade
- Year taken / grade level
- Credits (.5 or 1)
- Special designation if applicable (Honors, Dual Enrollment, Community College)
Examples:
Algebra I / 91 / A- / 9th grade 2020 / 1 CreditSpanish 2 / 95 / A / 11th grade 2022 / Community College (DE) / 1 CreditTest Scores (if applicable)
Include any college entrance exam scores (SAT, ACT, etc.)
